The song is the last verse of the Avinu Malkeinu prayer which is recited daily from Rosh Hashanah to Yom Kippur during the Ten Days of Repentance.
On Jewish New Year - Rosh Hashanah - it is sung as part of the Shacharit (morning) service.
On the Day of Atonement - Yom Kippur - it's recited in the Shacharit (morning) service and at Neila, the closing service.
This prayer is omitted if Yom Kippur falls on the Sabbath.
